What is citrulline malate, and how does it work?
Citrulline malate is L-citrulline, a non-essential amino acid, bound to malate at a 2:1 ratio. In the body, L-citrulline converts to arginine, which raises nitric oxide — a molecule that widens blood vessels and improves blood flow. Malate is a Krebs-cycle intermediate involved in energy production.
The chain is simple. You swallow L-citrulline, it slips past the gut and liver, the kidneys turn it into arginine, and arginine feeds the enzyme that makes nitric oxide. More nitric oxide means wider vessels and more blood, oxygen and nutrients reaching working muscle. That is the whole proposed mechanism for the training benefit, and a 2023 review of nitric-oxide precursors lays it out the same way. [5]
Malate is the other half. It sits inside the citric-acid cycle your cells use to make energy, and the theory is that adding it helps recycle ATP during hard sets. That part is more hopeful than proven — no clean trial has separated a malate effect from the L-citrulline effect. So when you read about citrulline malate, picture an effective nitric-oxide precursor with a plausible-but-unconfirmed energy sidekick attached.
Does citrulline malate actually work?
Modestly, and the evidence is genuinely mixed. Two meta-analyses found small benefits — Trexler 2019 reported a small effect on strength and power, and Vårvik 2021 found about three extra reps to failure, a 6.4% increase. Some single trials show a bigger effect; several others show nothing at all.
Start with the strongest evidence. Trexler and colleagues pooled 12 studies on high-intensity strength and power and found a small but statistically significant benefit, with an effect size around 0.20. [1] Vårvik's team then looked specifically at reps to failure across 8 studies and 137 lifters, and landed on roughly 3 extra reps overall — real, but small. [2] Both papers are careful to call the effect modest, not dramatic.
The study that launched the hype is Pérez-Guisado 2010: 41 men took 8 g of citrulline malate and did about 52% more reps in the later sets of a bench-press workout, plus reported roughly 40% less soreness at 24 and 48 hours. [3] That is a big result — but it is one study, and the meta-analyses that followed averaged in plenty of trials showing no change in 1RM, isometric force or power. The honest read: a small, inconsistent edge on higher-rep work, not a sure thing.

What about the "pump" everyone talks about?
Citrulline malate does produce a bigger pump. By raising nitric oxide, it widens blood vessels so more blood reaches the muscle you are training, which makes it feel fuller and look more vascular during the session. The pump is real but temporary, and it is a weak predictor of long-term growth.
The muscle pump is the swollen, tight feeling you get from blood and fluid gathering in a working muscle. Because nitric oxide is a vasodilator, more of it means more blood flow and a more noticeable pump — this is the sensation most people are actually paying for with a pre-workout. It feels great and it can be motivating.
Here is the honest caveat: a good pump looks impressive for an hour and then it is gone. Some researchers think cell swelling might nudge growth signalling a little, but the link between acute pump and real hypertrophy is loose. Do not mistake the pump for progress. Progress is more weight or more reps on the bar over weeks, which you can see plainly by logging your sessions rather than judging by how veiny you looked mid-workout.
How much should you take, and when?
Use 6-8 g of citrulline malate about 40-60 minutes before training — that is the dose and timing behind most positive studies. If you prefer pure L-citrulline, 3-6 g is the equivalent. Timing matters here because you want blood arginine and nitric oxide to peak while you actually lift.
The 40-60 minute window comes from how quickly L-citrulline raises blood arginine after you take it. Gonzalez and Trexler's review recommends this pre-exercise timing and dose range directly. [4] Take it on a relatively empty stomach so absorption is not slowed by a big meal. Unlike creatine, which works by slowly saturating muscle over days, citrulline's benefit is acute — so when you take it genuinely matters.

L-citrulline vs citrulline malate vs arginine
L-citrulline is the ingredient that works. It raises nitric oxide more reliably than arginine because it survives digestion instead of being destroyed on the first pass through the gut and liver. Citrulline malate is simply L-citrulline plus malate; both forms deliver the same effective L-citrulline.
For years arginine was the go-to nitric-oxide supplement, but most of an oral arginine dose gets broken down before it ever reaches your blood. L-citrulline sidesteps that, then converts into arginine inside the kidneys — so paradoxically, taking citrulline raises blood arginine better than taking arginine does. Between L-citrulline and citrulline malate, the choice is minor. Citrulline malate has the classic rep-performance studies behind it [3]; pure L-citrulline is often cheaper per gram of active ingredient and tends to taste less sour. Pick on price and label transparency, and do not pay a premium for the malate on its own.
Is citrulline malate safe?
For healthy adults, citrulline malate is well tolerated. Trials report no serious side effects, with mild stomach discomfort being the most common complaint at higher doses. The main real caution is blood pressure: because L-citrulline can lower it slightly, people on relevant medication should check with a doctor first.
Across the resistance-training studies, participants took 6-8 g without meaningful problems. [4] The most likely issue is a bit of gut upset if you take a large dose on an empty stomach — easily fixed by lowering the dose or having a small snack. There is no known dependency, no need to cycle it, and no evidence of harm from daily use in healthy people.
The one group that should be careful: anyone taking blood-pressure medication, nitrates, or erectile-dysfunction drugs, since citrulline's mild blood-pressure-lowering effect can add to theirs. If that is you, or you are pregnant, ask your doctor before starting. As always, a supplement is a small addition on top of training, food and sleep — never a substitute for them.
Who might benefit, and who won't notice?
Citrulline malate is most likely to help lifters doing higher-rep, pump-style resistance work — think 8-20 reps per set, bodybuilding-oriented training. Powerlifters doing heavy singles and triples, and anyone chasing a maximal 1RM, are least likely to feel it, because the evidence sits on rep endurance, not peak strength.
The mechanism explains the split. More blood flow helps most when you are grinding out reps and clearing metabolic byproducts, which is exactly the higher-rep zone. That is also where the positive studies were run — sets taken to or near failure. [2] [3] If you mostly do low-rep max-effort work, the muscle spends less time in that blood-flow-limited state, so citrulline has less to offer.
Endurance athletes get a small, separate signal: Suzuki's 2016 study found L-citrulline improved a 4 km cycling time trial by about 1.5%. [6] It is a minor effect and less studied than the lifting side. Citrulline is also a genuinely useful option for people who want a stimulant-free boost — it stacks cleanly with beta-alanine and creatine, and unlike caffeine it will not keep you awake if you train at night. Should you take it? The verdict, and common myths
Citrulline malate is a reasonable maybe, not an essential. The evidence points to a small, inconsistent benefit — a few extra reps and possibly less soreness — at a low cost and with a clean safety profile. If your training, protein, sleep and creatine are already handled, it is a sensible thing to test. If they are not, fix those first.
My coaching take after running it with a lot of lifters: some feel a clear difference on high-rep days, others feel nothing, and the logbook usually shows a small edge at best. That matches the research perfectly — real but modest. It will not build muscle for you, and it will not turn an average session into a great one. It might buy you a couple of reps you would not otherwise get, and over months those reps add a little volume.
- "Citrulline builds muscle." — It does not build muscle directly. At best it lets you grind out a couple more reps, which adds a little training volume over time. The muscle comes from the extra work, not the powder.
- "A bigger pump means bigger gains." — The pump is transient vasodilation and cell swelling. It looks great in the mirror for an hour but is a weak, unreliable predictor of long-term hypertrophy.
- "The 2:1 malate ratio is a performance secret." — Malate is a normal energy-cycle molecule, but no good trial isolates a clear malate benefit over plain L-citrulline. The ratio is mostly branding.
- "More is always better." — Past roughly 8 g of citrulline malate you mainly buy stomach discomfort, not more reps. The studied doses top out around there for a reason.
Bottom line: treat citrulline malate as a low-stakes experiment. Take 6-8 g, 40-60 minutes before a few high-rep sessions, and watch whether your reps on the log actually climb. If they do, keep it.
