The kidney fear comes from a single misunderstood marker. Creatine is metabolised into creatinine, and doctors use blood creatinine as a rough proxy for kidney function. Supplementing can nudge that number up slightly — not because anything is being damaged, but simply because you are eating more of the precursor. A 2023 narrative review looked squarely at the case reports behind the "creatine causes kidney failure" idea and concluded that controlled trials do not support it. Two honest caveats. If you already have kidney disease or a diagnosed kidney condition, talk to your doctor before starting — the reassurance above is for healthy people. The hair-loss one deserves a specific mention because it is so sticky. It rests entirely on a 2009 study of college rugby players that found a rise in the ratio of DHT to testosterone during a creatine loading phase — a hormonal marker, not actual hair loss, and a result no one has reproduced since. [7] No trial has ever measured creatine causing baldness. The cramp and dehydration fears fare no better: reviewed directly, creatine shows no increase in cramping, and some data suggest it may reduce it.
